- I understand that my child is registered and paid for as a participant in one of the Treehouse Summer Classes.
- I understand that my child must be well each day, with no fever, cough, nausea, or other illness.
- I understand that my child must wear a mask to class each day and must wear it correctly during each session.
- I agree to arrange for my child to be brought to class 10 minutes prior to the start of class, and picked up each day promptly at the end of class. If my child is to be picked up by someone other than myself, I have indicated that name as my emergency contact and provided a phone number.
- I agree that my child will be dressed appropriately and be ready to participate in class.
- Treehouse staff and volunteers are happy to accommodate learning styles and abilities as much as possible within the framework of the class. I understand that disruptive behavior is a sign my child does not wish to participate in the class. I will be notified by Treehouse if such behavior takes place and I agree to remove my child from the class with no refund of fees.
- I also understand that Treehouse is a drug, alcohol and tobacco free facility and I agree that my child and I will respect and follow that policy.
- I understand that Treehouse, and those hired by Treehouse to conduct classes, will do everything possible to ensure the safety and security of my child while they are at Treehouse. I understand that they will do everything possible to make the class enjoyable and educational for my child.
- Often for grant reporting, media coverage, or archive purposes, photographs are taken during classes and workshops at Treehouse. I give permission to Treehouse Museum to use photographs and/or video and/or audio of my child and me obtained while my child is enrolled in classes or workshops at the Museum. I release all persons connected with or affiliated with Treehouse Museum from any and all liability arising from the use of these photos or videos for publicity purposes and waive the right to all negatives, photos, tapes, and reproductions, as well as waive the right to inspect or approve the finished photographs and/or recordings.
- By clicking below, as the parent representing my child, I expressly waive and hereby release all persons connected with or affiliated with Treehouse Museum from any and all liability to my child in connection with the class activities for the Museum by virtue of this agreement with the Museum.
